Life Care Planning

Whether you are caring for your loved one in the home, trying to make a decision on appropriate living arrangements or need additional information on the “puzzle” of long term care, you know how difficult, time-consuming and isolating care giving can be.
Imagine what life would be like if you had a team of compassionate advisors and expert professionals helping you obtain the right care, preserve family resources and help make those difficult decisions? Wouldn’t you like the peace of mind that results from loved ones enjoying the best quality of life possible?  That’s what life is like when you work with Weatherby & Associates, PC, a Life Care Planning Law Firm, who can help develop a workable care plan for loved ones, maximizing their available resources and even find additional resources such as the Veterans Aid & Attendance benefit.

What is Life Care Planning?
Life Care Planning is a holistic, elder-centered approach to the practice of law. The goal of Life Care Planning is to promote and maintain good health, safety, well-being, and quality of life of seniors and their families. Seniors and their families get access to a wider variety of options for care as well as knowledgeable guidance from a team of compassionate advisors who help them make the right choices about every aspect of their loved ones well-being.  Weatherby & Associates, PC relies on an inter-disciplinary team that works to identify present and potential future care needs.
Weatherby & Associates, PC is one of approximately 150 firms nationwide who has joined the Life Care Planning Law Firms Association.
Managed by our experienced attorneys and our Gerontologist, Denise Talbot. A Life Care Plan helps you respond to every challenge created by the long-term illness or disability of your loved one before, during and after the onset of declining health.
A Life Care Plan bundles the right combination of estate planning, asset protection, legal services, public benefits qualification, and assistance with residential placement, family education, and health care advocacy. This approach relies less on crisis-oriented transactions and more on the development of on-going relationships with families. In addition to our staff, we are founders of the Connecticut Care Planning Council, a group of unaffiliated companies and individuals who furnish services to elders and their families.

Initial Meeting
Your initial meeting with Weatherby & Associates, PC, will focus on your loved one’s health, mobility, housing and financial resources. After an in-home assessment (residential, assisted living, independent or nursing or independent living facility) by the gerontologist, gaps in care can then be identified and solutions discussed for closing those gaps. When your customized Life Care Plan is developed, it defines, organizes, prioritizes, and mobilizes every aspect of your loved ones care, to allow him or her to maintain the quality of life that he or she desires.
Don’t shoulder care giving challenges alone.

Had we known how painless he would make the process, we would never have procrastinated. My husband and I engaged Hank Weatherby's services last year to help us plan for our children and grandchildren...
